Terms of Service
Understand the service, your responsibilities and how subscriptions and account closure work.
Using FormShark
FormShark lets you create and share surveys, collect responses and manage results. This draft describes the web service. Mobile store releases are not yet available. The operator is a person based in the United Arab Emirates; the operator’s legal name and service address must be confirmed before these terms take effect. General questions can be sent to support@formshark.app and billing questions to billing@formshark.app. These contact routes are configured, with delivery testing still pending.
Plans and subscriptions
Free: 3 active surveys and 100 accepted responses per month.
Pro: USD 10 per month, 25 active surveys and 2,000 accepted responses per month; logo, colour customisation and FormShark branding removal.
Business: USD 20 per month, unlimited active surveys, 10,000 accepted responses per month and 10 GB of owner-paid media storage. Includes Pro branding features and the read-only API.
These are the approved monthly plans; no new annual offer is made. Existing paid unlimited allowances are retained. Actual purchase availability and the payable amount must be shown in checkout; listing a plan here does not establish that its payment setup is live.
Paid monthly subscriptions renew unless you cancel through Account → Manage subscription. Review the provider’s confirmation and paid-through date. Cancelling renewal and deleting an account are separate actions. Tax treatment and refund/dispute entitlement rules remain launch decisions; this draft does not establish a no-refund rule or remove rights provided by applicable law.
Your surveys and content
You retain your rights in the content you provide. You permit FormShark to store, process and transmit it to provide the features you choose, including sharing surveys and delivering configured integrations. Only upload content you are authorised to use and collect responses you are entitled to collect. Tell respondents who is collecting their information, why, and how to contact you.
Do not use the service for unlawful content, infringement, unauthorised access, credential theft, malicious code or interference with other users. Protect account credentials and integration secrets. The final suspension, notice and appeal process requires operator approval before launch.
Media, integrations and offline work
Business media uses existing image, video and audio files. Storage is charged to the survey owner’s allowance. The Business API reads survey data; it does not create or change surveys or responses. Webhooks are available on every plan and send data to destinations chosen by the survey owner. See the integration guides before connecting another service.
Offline drafts and queued responses can remain on your device until synchronisation. Check that submission has completed before clearing browser data or changing devices. Exported files and copies held by recipients or external integrations are outside FormShark’s direct control.
Cancellation and account deletion
You must cancel any subscription yourself before requesting account deletion. FormShark does not automatically cancel it. Server verification can block deletion while billing remains unresolved. Account deletion is currently disabled pending policy, backup and operational checks.
The approved target is to delete profile, survey and media content within 30 days of an accepted request, subject to verification before enabling the feature. This is not a guarantee of backup erasure. Minimal payment references, cancellation evidence and deletion-status records are separate; their retention duration is undecided.
Terms still requiring approval
Before publication: confirm the contracting operator and test email delivery; effective date and acceptance/change-notice process; tax and invoice treatment; refunds and disputes; retention duration; service suspension and termination; any warranty/liability provisions; governing law and dispute process.
