Sign-in and recovery

For an email/password account, choose Forgot password on the sign-in screen. Check your inbox and spam folder. An expired or used reset link requires a new request. For Google or Apple sign-in, recover access with that provider, then return to FormShark.

Surveys, responses and offline work

Open your survey to edit its questions or view responses. Check your plan’s usage in Account if a survey or response limit is reached. On a shared device, check for unsent drafts and queued responses before clearing app data. Reconnect and confirm synchronisation before assuming another device has the latest results.

Subscriptions and payments

Open Account → Manage subscription to manage or cancel renewal. Keep the provider’s cancellation confirmation. Cancelling renewal does not submit an account deletion request. Refund and dispute entitlement policy is still under review; this page makes no promise of a refund and does not impose a no-refund rule.

Delete your account

Deletion is currently disabled pending policy and operational checks. The account deletion page explains what will be removed, separate retained records, cancellation prerequisites and how a request will be tracked when enabled. It is not confirmation that requests are currently accepted.

Email support@formshark.app for help, privacy@formshark.app for privacy questions, or billing@formshark.app for subscription and payment questions. These links open your email app; this page does not submit a request. The support mailbox and forwarding addresses are configured, but send/receive delivery has not yet been tested.

Include the affected feature, time of the issue, device/browser and any error text. Do not send passwords, API secrets, payment card details or unnecessary respondent data.
