Privacy Policy
How FormShark handles account information, survey responses, uploaded files and service records.
Who handles your information
FormShark is operated by a person based in the United Arab Emirates. The operator’s legal identity and service address are not yet confirmed for publication. Privacy questions can be sent to privacy@formshark.app; delivery testing is pending. This prevents this draft from serving as a finished privacy notice.
Survey owners choose the questions, recipients and integrations for their surveys. If you are a respondent, contact the survey owner about their collection and use of your answers. Deleting your FormShark account does not by itself identify or erase answers you submitted to another owner’s surveys.
Information used by the service
- Account: authentication identifiers, email and available profile name/photo or provider information, used to sign you in and show your account.
- Surveys and responses: question definitions, settings, answers, timestamps and ownership/access records, used to collect and display results.
- Media: uploaded image, video and audio files and associated file metadata, used to attach media to responses. Files may contain personal information or embedded metadata; inspect them before uploading.
- Billing: customer/subscription references, payment status and usage records, used to manage plan access, quotas and payment reconciliation. Checkout is handled by Stripe; this draft does not claim that processor records are erased with your account.
- Integrations and operations: credentials, webhook destinations/delivery records and deletion job status, used to authenticate integrations, troubleshoot delivery and complete account cleanup.
The final notice must confirm infrastructure log fields and retention, all production SDKs and any analytics or cookies before release. A source review is not a completed live data audit.
Data on your device
The app supports local persistence, drafts and queued responses to support offline work. Signing out is not a promise that all device copies are erased. Account cleanup clears owned local records and app caches on the requesting device; drafts for other owners may remain because they cannot be attributed to your account. Clear app data separately on other devices, after checking for work you still need.
Retention and deletion
Account deletion is currently disabled pending configuration, deployment and operational verification. The approved target is profile, survey and media deletion within 30 days of an accepted request. Backup, soft-delete and other recoverable-copy handling has not been verified; no 30-day backup purge is guaranteed.
Approved retained records are minimal payment references, cancellation evidence and deletion-status records, used for reconciliation/disputes, proving cancellation and safely completing cleanup without recreating deleted data. Their retention period and eventual deletion process remain undecided. No seven-year period has been selected. Provider-held billing records are separate.
Read deletion availability, scope and steps. You must cancel subscriptions yourself first; the service verifies cancellation and does not cancel on your behalf.
Questions and privacy requests
You can review your profile and survey content in the app, export available results and manage connected integrations. Use password recovery for an email/password account or your provider’s recovery flow for Google/Apple sign-in. Email privacy@formshark.app for privacy questions. This configured address forwards to the support mailbox; send/receive delivery testing is pending. The link opens your email app, and this page does not submit requests.
The final notice must explain applicable privacy rights, lawful grounds for processing, how to exercise rights and complain, response handling, age/children policy and security practices. These details require operator and legal review; no response deadline or compliance certification is claimed.
